Sessions are by appointment only. Our office accommodates our client’s schedules and offer early morning, day and evening appointments in addition to limited hours on Saturday.  The office is closed on Sundays. Initial sessions are schedule for 1hr. Each session typically lasts for 45-50 minutes.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.  Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

 ACCEPTED PAYMENTS
Payment is due at the time of service.  We accept cash, check or credit card. 

CANCELLATION POLICY

Each client is required to maintain an active and valid credit card on file, which will enable us to bill automatically for any payments due after each date of service. This will assist with billing procedures, as well as prevent the accumulation of outstanding balances. If your card is declined, you are required to immediately update the card information. Failure to do so will result in cancellation of services.

Please give your therapist 24 hours notice for any appointment you will need to cancel (leaving a voice mail notification for your therapist is acceptable). Because each appointment time is reserved specifically for you, it is necessary to charge a
late cancellation fee of $50.00 for appointments which are cancelled with less than 24 hours notice. The same fee will apply if you fail to show for a scheduled appointment without calling to cancel. If you are using insurance to pay for your psychotherapy services, please be aware that your insurance will not pay for a missed appointment or late cancel fee. 

Please note that your card on file will be charged AUTOMATICALLY if an appointment is considered a “Late Cancel” or “No Show”. Reminder: If you fail to attend your session on time (more than 15 minutes late without notification) we count this as a “No Show”

REQUESTS FOR COURT APPEARANCES
It is the policy of APOM that we dispute the counselors schedules as little as possible as being available to clients for their sessions is of the utmost importance to us. 

Therefore, it is our policy that any court appearance is charged a flat fee of $1500.00 per day.  Additionally, one-half of the fee is due upon the receipt of a subpoena and the balance is due the day before the court appearance.

DOCUMENTATION REQUESTS
APOM is happy to fulfill requests made for documentation.  There is a
$25 documentation fee and all requests will be completed within 14 business days. If a request for receipt is less than 14 business days, a $50 expedited fee.
